Flat sheet, fitted sheet, or both
A fitted sheet goes over the mattress and stays there. A flat sheet lies between you and the duvet, which keeps the duvet cover clean for longer and gives you a layer to shed on a warm night. Plenty of people sleep happily without one; plenty would not consider it.
Why linen rather than cotton
Cotton feels smoother on night one. Linen feels better on night three hundred. The fibre is hollow, so it breathes and regulates temperature in both directions, and it grows softer with every wash instead of thinning the way cotton does. It is also the stronger of the two fibres by a wide margin.
Care
Machine wash in cool to warm water with a gentle pH-balanced detergent, avoid bleach and harsh chemicals, then air dry or tumble on low. Take it off the line slightly damp and the creases settle themselves.
